ABSTRACT

Objective:To study the clinical medication regulation for the prevention and treatment of new coronavirus pneumonia with traditional Chinese medicine(TCM),especially prescriptions,on the basis of "treatment in accordance with seasonal conditions". Method:The guidelines and suggestions on novel coronavirus pneumonia formulated and published by national,provincial and municipal governments and experts before February 17,2020, were retrieved and summarized. Data was recorded in stages according to "prevention period,clinical observation period,pre-clinical period,middle clinical period,late clinical period and recovery period". The frequency analysis of TCM and its efficacy,prescription and proprietary Chinese medicine was carried out,and then the factor analysis and cluster analysis of TCM were carried out to obtain rational drug combinations. Result:Totally 172 suggestions on combined structure of drugs,50 suggestions on prescriptions and 31 suggestions on proprietary Chinese medicine were involved in the study,and provided by 24 official agencies and 24 medical experts, 147 kinds of herbs,44 prescriptions and 16 proprietary Chinese medicines were collected. No matter for the drug category or for the specific drug selection,the law of drug use in different stages was not the same. Huoxiang Zhengqi capsule,Shufeng Jiedu capsule,Lianhua Qingwen capsule and Jinhua Qinggan capsule can be used in the observation period. Xuanbai Chengqitang can be used in the middle of the disease. Angong Niuhuangwan,Suhexiangwan or Zixuedan, and Angong Niuhuangwan can be selected in the middle of the disease. And Qingfei Paidu decoction can be used in all stages of the disease. Conclusion:Doctors need to adjust their prescriptions along with the progress of the disease,because the law of medication in each stage of the new coronavirus pneumonia is different.

ABSTRACT

Objective To evaluate the accuracy of human brucellosis diagnosis and reporting in medical institutions in Shanxi province, and understand the performance of clinical doctors to diagnose human brucellosis according to diagnostic criteria. Methods Field investigation was conducted in 6 medical institutions in the key areas of human brucellosis in Shanxi province. The diagnosis data of the reported brucellosis cases in 2015 were collected and reviewed retrospectively for the evaluation of the diagnosis accuracy with systematic sampling method. The database was established with Excel 2010 and the descriptive analysis and statistical test were conducted with software R 3.3.2. Results The diagnosis consistent rate of the 377 brucellosis cases reviewed was 70.8% (267/377), the diagnosis consistent rates in medical institutions at city-level and country-level were 77.0%(127/165) and 66.0%(140/212) respectively, the differences had significance (χ2=5.4, P=0.02). Among the reviewed cases, the diagnosis consistent rate of laboratory diagnosis and clinical diagnosis were 87.1%(256/294) and 13.3%(11/83) respectively, and the differences had significance (χ2=170.7, P<0.001) . Among the 21 investigated clinical doctors, the numbers of the doctors who correctly diagnosed the suspected cases, probable cases and lab-confirmed cases were only 3, 0 and 8 respectively. All of the clinical doctors knew that it is necessary to report the brucellosis cases within 24 hours after diagnosis. Conclusion The accuracy of human brucellosis diagnosis in key areas of human brucellosis in Shanxi was low, and the performance of the clinical doctors to diagnose human brucellosis according to diagnostic and case classification criteria was unsatisfied.

ABSTRACT

E2A is involved in promoting forkhead box P3 (FOXP3) and retinoid-related orphan receptor gamma t (RORγt) gene transcription,which are pivotal transcription factors of T regulatory cells and Thl7 cells,respectively.Little is known about the involvement of E2A in pregnancy process.This study aimed to investigate the expression of E2A,cytotoxic T-lymphocyte-associated protein 4 (CTLA-4),and Foxp3 in luteal phase endometrium of women suffering recurrent miscarriage (RM) (n=21) and control group (n=11) by immunohistochemistry,with the Vectra(R) automated quantitative pathology imaging system for analysis.The percentage of E2A+ cells and CTLA-4+ cells was significantly higher in the endometrium of women with RM than in the controls.There was positive correlation between E2A and CTLA-4 (r=0.523,P=0.002),E2A and FOXP3 (r=0.380,P=0.032),and FOXP3 and CTLA-4 (r=0.625,P=0.000) in the mid-secretory phase of endometrium for all subjects.It was concluded that the abnormal expression of endometrial E2A existed in mid-secretory endometrium of women with RM,and there was a positive correlation between E2A and FOXP3,and E2A and CTLA-4,suggesting the possible regulation role of E2A involved in regulating endometrium receptivity.

ABSTRACT

Objective To compare biomechanical characteristics of the knee joint during forward walking and backward walking. Methods Temporal-spatial, kinematics, kinetics parameters of 13 healthy young male volunteers were collected and compared by 3D motion capture system Vicon T40 and force platforms AMTI OR6-7. Results Compared with forward walking, the speed, cadence and stride length significantly decreased, while the gait cycle and stance phase percentage in gait cycle significantly increased during backward walking. In the sagittal plane, the range of motion (ROM), the maximum flexion/extension moment of the knee were smaller during backward walking. In the frontal plane, the ROM of knee varus/valgus during backward walking decreased, and the peak value of knee adduction moment significantly reduced in the early stance phase while significantly increased in the late stance phase of backward walking. The peak value of ground reaction force (GRF) was significantly larger in the early stance phase while smaller in the late stance phase during backward walking than that during forward walking. Conclusions The biomechanical characteristics of the knee joint during forward walking and backward walking are significantly different. Compared with forward walking, backward walking is helpful to reduce the medial compartment load in the early stance phase. Further study will be needed to investigate the effects of backward walking on knee joint loading in the late stance phase.

ABSTRACT

Objective To find out whether people in Xinzhou Shanxi know the hazard of both iodine deficiency disorders and drinking-water-borne endemic tluorosis,in Xinzhou Shanxi and to promote people actively participate in prevention of the diseases and to achieve a sustainable development of health education in endemic diseases.Methods Ningwu,Baode,Kelan and Jingle Counties were selected to carry out the.health education program of iodine deficiency disorders and Wutai,Xinfu and Fanshi counties were selected to carry out the health education project of drinking-water-borne endemic fluorosis in Xinzhou City Shanxi Province in 2011.To carry out the base line questionnaire survey,in the beginning and at the end of the project,three townships from each county were chosen,and one primary school was selected from each chosen township,15 housewives were selected from each chosen township and 30 fifth grade students were selected from each primary school.Results ① The baseline survey:a total of 366 pupils of grade 5 and 183 housewives were investigated,and the awareness rate of iodine deficiency disorders was 77.41% (850/1098) and 80.33% (441/549),respectively; a total of 270 pupils of grade 5 and 138 housewives were investigated,and the awareness rate of drinking-water-borne endemic fluorosis was 80.74% (654/810) and 81.40% (337/414),respectively; ② The effect evaluation:a total of 366 pupils of grade 5 and 181 housewives were investigated,and the awareness rates of iodine deficiency disorders were 91.62% (1006/1098) and 92.45% (502/543),which were compared with that of baseline investigation,and the awareness rates were improved significantly (x2 =84.69,34.04,all P < 0.01); a total of 270 pupils of grade 5 and 138 housewives were investigated,awareness rates of drinking-water-borne endemic fluorosis were 93.95% (761/810) and 93.48%(387/414),which were compared with that of baseline survey,and the awareness rates were improved significantly(x2 =63.94,27.47,all P < 0.01).Conclusions After implementation of health education project on endemic diseases,the self-protection awareness of the fifth grade students and housewives is promoted effectively,awareness of prevention knowledge on endemic diseases is raised,which lays a foundation for controlling and eliminating the endemic diseases.

ABSTRACT

Hyperglycemia is an important initiator of cardiovascular disease, contributing to the development of cardiomyocyte death and diabetic complications. The purpose of the present study was to investigate whether high glucose state could induce apoptosis of rat cardiomyocyte cell line H9c2 through microRNA-mediated Bcl-2 signaling pathway. The expression of miR-34a and Bcl-2 mRNA was detected by using real-time PCR. Western blotting was used to examine the changes in apoptosis-associated protein Bcl-2. Apoptosis of H9c2 cells was tested by using flow cytometry. The results showed that the expression of miR-34a was significantly elevated and that of Bcl-2 was strongly reduced, and apoptosis of cardiomyocytes was apparently increased in the high-glucose-treated H9c2 cells as compared with normal-glucose-treated controls. In addition, we identified Bcl-2 gene was the target of miR-34a. miR-34a mimics reduced the expression of Bcl-2 and increased glucose-induced apoptosis, but miR-34a inhibitor acted as the opposite mediator. Our data demonstrate that miR-34a contributes to high glucose-induced decreases in Bcl-2 expression and subsequent cardiomyocyte apoptosis.

ABSTRACT

<p><b>AIM</b>To investigate the effects of a nutritional supplement on nutritional status and hypoxia endurance in young adults living at high altitude.</p><p><b>METHODS</b>Forty healthy male young adults were recruited and randomly assigned to control and intervention groups. The nutrition survey was carried out using weighing method. The intervention group was given a nutritional supplement specifically designed for use at high altitude, while the control group was treated with a supplement made of stir-fried flour. After 20 days of supplementation, they marched from the altitude of 3700 m to 5100 m. The changes in HR, SaO2, serum concentrations of VA and VB2 and some minerals were measured.</p><p><b>RESULTS</b>The results of nutrition survey showed that the ratio of three macronutrients was not adequate and the intakes of calcium, VA and VB2 were below Chinese RNI. The serum concentrations of calcium, magnesium and VA were below normal references. The serum VB2 concentration was at the low level o f normal reference. The nutritional supplement could increase the serum concentrations of calcium, magnesium, VA and VB2, indicating an improved nutritional status. The changes in HR and SaO2 were diminished in intervention group compared with control group.</p><p><b>CONCLUSION</b>The nutritional supplement can improve nutritional status and increase the hypoxia endurance in young adults living at high altitude.</p>

ABSTRACT

<p><b>AIM</b>To investigate the effects of hypoxia on the secretions of proinflammatory cytokines TNF-alpha and IL-6 and to inquire into the mechanism.</p><p><b>METHODS</b>Separated mice abdominal macrophages which were identified with non-specific esterase dye method, and created the hypoxic cultured model. The levels of TNF-alpha and IL-6 in the medium were determined by ELISA method. The mRNA expressions of TNF-alpha and IL-6 were measured by RT-PCR method. NF-kappaB activation was assayed by Western blot method. Finaly, we added cortone (5 microg/ml) to the medium, then observed the secretion levels of TNF-alpha and IL-6 during hypoxia.</p><p><b>RESULTS</b>The secretions of TNF-alpha and IL-6 from Mphi exposed to hypoxia for 12 h were increased significantly compared with control (P < 0.01). The expressions of TNF-alpha mRNA and IL-6 mRNA were enhanced obviously contrasted with control (P < 0.01). NF-kappaB activation in Mphi nuclei was raised at 2 h during hypoxia and persisted to 5 h. We added cortone to the medium and found no significant change in secretion of TNF-alpha and IL-6 during hypoxia.</p><p><b>CONCLUSION</b>Hypoxia could activate NF-kappaB and make it shift to nucleus which promoted the transcriptions and expressions of TNF-alpha and IL-6.</p>

ABSTRACT

The effects of hypoxia on the level of reactive oxygen species (ROS), IkappaBalpha tyrosine phosphorylation, transcription of P65 mRNA and NF-kappaB activation in isolated rat peritoneal macrophages were investigated by DCFH-DA fluorescence spectrophotometry, Western blotting and RT-PCR. The results obtained are as follows. (1) During hypoxia, the levels of intracellular ROS began to increase at 1 h, then reached a peak at 2 h, and began to decrease after 3 h. IkappaBalpha tyrosine phosphorylation began to rise after 2 h hypoxia and was the highest after 3 h hypoxia. After 4 h hypoxia it decreased gradually. NF-kappaB activation began to increase after 3 h hypoxia, and reached a peak after 4 h hypoxia. (2) When antioxidant NAC (500 mmol/L) was added into the medium, the level of IkappaBalpha phosphorylation showed no significant changes during hypoxia. After adding protein tyrosine kinase inhibitor genistein (200 micromol/L), NF-kappaB activation induced by hypoxia was blocked significantly. (3) The expression of p65 mRNA was also elevated markedly during hypoxia. These results suggest that hypoxia may lead to IkappaBalpha phosphorylation and NF-kappaB activation through intracellular ROS, and that the regulation of NF-kappaB activity may involve IkappaBalpha phosphorylation and the expressions of each subunit gene of NF-kappaB.
